practicelaw Forms | Informal and Formal Probate Matters
Since May 2007, practicelaw's attorneys and staff have worked with a volunteer group of probate professionals to research, draft, edit and create a core set of 40 forms for use in most informal and formal probate matters. As of January 9, 2008, these are available in Microsoft Word format for download and use. Special thanks and recognition are due to a number of MSBA members and probate professionals for the development of these forms.
Practitioners have a choice. Click on the practicelaw Forms link above
or prepare them using the new MSBA::mndocs
document assembly system.
The MSBA continually improves forms based on changes in the law and user comments. To see the latest improvements to a particular form, visit the Changes and Improvements page.
Court Forms | Minnesota Judicial Branch
The Minnesota judiciary maintains a limited number of probate forms on its web site. Probate forms available from the state court site are generally intended for pro se parties.

practicelaw also maintains a comprehensive set of guardianship and conservatorship forms in its Guardianship/Conservatorship area of the site. Drafted and overseen by experienced attorneys, these forms are approved for use by the Minnesota Conference of Chief Judges.
