When this ep opens, Sam is the only one conscious on account of his body was shielded from The Impact by The Dad's and, symbolically, by Dean's. This should set you up to realize what a very, very good ep this is even though it never won an Emmy. (It should have, I tells ya!) It contains, to misquote the hooker from Risky Business, "What all the fans on the lakeshore want." It's got angst and darkness, sacrifice and love, brotherly conversations and fatherly silences.
